Ontada Health eRegistration (eReg) helps you fill out forms and other information before you see your provider.
eRegistration is easy to use on any smartphone, tablet, or computer with Internet. You don't need to download an app, sign up, or remember a password.
How It Works
On the day you schedule your appointment, your clinic sends you an email. The email has a link for you to fill out some forms online before your next visit.
To spend less time waiting at the clinic, safely enter your information whenever you want.
After you fill out each form, the clinic checks them and adds them to your online chart.
